About the role

Join Iconic Care, a physician-owned group of skilled providers (physicians, PAs, and NPs) offering care across ten states, with plans for expansion to all 50 states. We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.

Day-to-Day Responsibilities

• Rounding at 2 Skilled Nursing Facilities, 1 facility located in Chico CA on Mondays, Wednesdays and Fridays and 1 facility located in Redding on Tuesdays and Thursdays (current schedule, days of the week subject to change).
• Your primary focus will be delivering specialized physiatry care to 27+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
• Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
• Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
• Cooking care and coordinating care, referring to other specialties or services for issues beyond the scope of physiatry.

Qualifications

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
• A genuine passion for providing professional, personalized care in an autonomous environment.
• Strong communication skills, with the ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting.

• Set patient load, with focused consulting visits for 30-35 patients/day after ramp-up.
• Full autonomy in patient care with direct impact on patient outcomes at multiple facilities.
• Immediate Hiring Timeline: We’re looking to make hiring decisions soon, with a start date in 4-6 weeks following insurance and Medicare credentialing.
