Duties
- Assists patients in simple prescribed exercises and range of motion.
- Maintains records to report and discuss observations with a supervisor or case manager.
- Provides patients with help moving in and out of beds, baths, wheelchairs or automobiles, and with dressing and grooming.
- Changes bed linens, washes and irons patients' laundry, and cleans patients' quarters.
- Performs a variety of duties as requested by client, including running errands, preparing meals, etc.
- Functions as a team member, collaborating with and responsible to the patient and the homecare team.
- Communicates observations of significant changes in patient status to supervisor.
- Attends Multi-Disciplinary Team Meetings and documents attendance.
- Participates in Agency’s other mandated activities as requested by Agency Director.
Requirements
- Graduation from an approved certification/program.
- Current, unencumbered state certification to practice as a Direct Care Worker (DCW).
- Professional experience required per state regulations.
- Copy of current CPR.
- Flexibility, organization, and the ability to exercise sound judgment.
Physical Requirements
- Ability to regularly, sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms.
- Frequent lifting and/or moving up to 15 pounds, occasionally lifting or moving 100 pounds or more if applicable.
- Ability to perform the anticipated level of physical assist required to provide safe client care.
Work Environment
- Highly variable work environment, generally provided in private homes.
- Exposure to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
