Phoenix ATC/ HAB

Duties

• Assists patients in simple prescribed exercises and range of motion.
• Maintains records to report and discuss observations with a supervisor or case manager.
• Provides patients with help moving in and out of beds, baths, wheelchairs or automobiles, and with dressing and grooming.
• Changes bed linens, washes and irons patients' laundry, and cleans patients' quarters.
• Performs a variety of duties as requested by client, including running errands, preparing meals, etc.
• Functions as a team member, collaborating with and responsible to the patient and the homecare team.
• Communicates observations of significant changes in patient status to supervisor.
• Attends Multi-Disciplinary Team Meetings and documents attendance.
• Participates in Agency’s other mandated activities as requested by Agency Director.

Requirements

• Graduation from an approved certification/program.
• Current, unencumbered state certification to practice as a Direct Care Worker (DCW).
• Professional experience required per state regulations.
• Copy of current CPR.
• Flexibility, organization, and the ability to exercise sound judgment.

Physical Requirements

• Ability to regularly, sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms.
• Frequent lifting and/or moving up to 15 pounds, occasionally lifting or moving 100 pounds or more if applicable.
• Ability to perform the anticipated level of physical assist required to provide safe client care.

Work Environment

• Highly variable work environment, generally provided in private homes.
• Exposure to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
