Responsibilities
- Captures and documents initial and ongoing assessments of patient, drug, and disease information.
- Escalates identified issues to clinicians as necessary.
- Answers patient, family, physician, and other healthcare professional inquiries.
- Schedules and coordinates IV therapy and home medical equipment orders.
- Picks and pulls supplies and prepares labels, prescriptions, and compounding/mixing worksheets.
- Maintains accurate documentation in patient records and pharmacy systems.
- Communicates supply inventories to the pharmacist and documents all interactions.
- Participates in meetings and quality improvement activities.
Qualifications
- Successful completion of an approved pharmacy technician training program; IV certification preferred.
- Registered Pharmacy Technician license in state of practice.
- National Pharmacy Technician Certification as required by the state board of pharmacy.
- One to three (1-3) years of related pharmacy experience in a home care setting, retail, or pharmacy setting.
- General knowledge of drug terminology, pharmaceutical calculations, and aseptic techniques.
- Effective mathematical and communication skills.
- Ability to work independently, accurately, and efficiently.
