About the role

The Pharmacy Coordinator is responsible for developing and implementing programs that support organizational goals around medication management, continuously improving the medication use process across the organization to deliver high-quality patient care. This role plans, designs, implements, and evaluates processes involved in medication selection/procurement, prescribing, preparation/dispensing, administration, and monitoring.

The Pharmacy Coordinator collaborates with medical staff, professional staff, and pharmacy staff to ensure a high-performing medication use process and quality outcomes. This position participates in developing and reviewing policies and procedures to optimize drug utilization, supports organizational goals through quality customer service, performance improvement efforts, and teamwork.

Responsibilities

  • Develop and implement programs to support medication management and improve the medication use process.
  • Plan, design, implement, and evaluate processes for medication selection, procurement, prescribing, preparation, dispensing, administration, and monitoring.
  • Collaborate with medical, professional, and pharmacy staff to ensure high-quality outcomes.
  • Participate in the development and review of policies and procedures to optimize drug utilization.
  • Provide quality customer service and participate in performance improvement efforts.
  • Demonstrate department-specific competencies as listed on the Assessment and Evaluation Tool.
  • Engage in educational activities for continued professional and personal growth.

Pharmacy Coordinator – Clinical

  • Provide Medication Therapy Disease Management (MTDM) services, including Anticoagulation, Pain Management, Diabetes Mellitus, Congestive Heart Failure, and Chronic Obstructive Pulmonary Disease.
  • Develop new clinical programs and projects aligned with organizational goals.
  • Provide supportive and clinical assistance to MTDM clinic services.
  • Manage specific initiatives assigned by Pharmacy Leadership.
  • Serve as co-chair of the Ambulatory Subcommittee.

Pharmacy Coordinator – Medication Safety

  • Serve as the authoritative expert in safe medication use and oversee the medication use system.
  • Organize and manage the organization’s medication safety initiatives and medication management policies and procedures.
  • Develop, implement, and monitor programs to continuously and safely improve medication use processes.
  • Provide guidance and support during regulatory surveys and inspections.
  • Manage specific initiatives assigned by Pharmacy Leadership.
  • Co-chair the Medication Safety Subcommittee of P&T (MedSTAR).
  • Serve as the Secretary of the Pharmacy and Therapeutics Committee and support P&T Chairmen.

Pharmacy Coordinator – Informatics

  • Lead all clinical technology projects, including strategy, vision, planning, and budget.
  • Develop, implement, maintain, and monitor processes for technology impacting Pharmacy Services, such as Cerner Millennium Electronic Medical Record, CPOE design, Pharmacy Inventory Management Systems, Automated Dispensing Cabinet processes, Bar-coding for Point of Care, and Clinical Decision Support for Prescribers, Pharmacists, and Nurses.
  • Advise the Pharmacy procurement team.
  • Provide IT support for contracted services within Pharmacies at Acuity Long Term Hospital (LTACH) and Bacharach Rehabilitation Hospital (BRH).

Pharmacy Coordinator – Innovation and Analytics

  • Coordinate, execute, navigate, and connect innovative tactics for Pharmacy Practice processes and programs across the Pharmacy Service Line.
  • Serve as a liaison for inter-professional and interdepartmental collaborations, disseminate information, and perform predictive analytics, financial analyses, and statistics.
  • Monitor and surveil clinical processes and programs to maintain high quality, performance, and value of the Service Line.
  • Oversee the upkeep and maintenance of all pharmacy service line scorecards, dashboards, and action plans in areas such as cost containment, savings, performance improvement, high-risk clinical processes, customer experience, and employee engagement.
  • Drive standardization and improve operational efficiencies using appropriate tools and techniques.
  • Perform duties to meet Service Line demands and other duties assigned by the Senior Director or AVP.

Work Environment

Potential exists for exposure to hazards and risks of the hospital environment, including infectious disease, hazardous substances, and potential injury. Requires the ability to work at a computer 60% of the time and some standing and walking.

Requirements

  • Graduate of an accredited school of pharmacy.
  • Doctor of Pharmacy degree or greater than 5 years of active pharmacy practice.
  • Current license in the State of New Jersey as a Registered Pharmacist.
  • Two years or greater of clinical pharmacy experience.
  • Excellent written and verbal communication, presentation, and interpersonal skills.
  • Excellent time management, organizational, and team-building skills.
  • Leadership skills to develop and implement clinical programs.

Preferred Qualifications

  • MTM certification or experience.
  • ACPE certification in disease-specific management.
  • Completed Pharmacy Postgraduate Year 1 or Board of Pharmacy Specialty certification.
