As a Pharmacy Clinical Specialist for BMT/Leukemia, a caregiver in this role will support oncology patients across both inpatient and outpatient settings.

Responsibilities

• Coordinate and provide pharmaceutical care for patients
• Participate in patient care rounds
• Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients
• Provide general and patient-specific drug information
• Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy
• Participate in daily interdisciplinary rounds
• Lead chemotherapy-related decision-making and Beacon order verification
• Manage discharge and consult services
• Collaborate closely with inpatient, outpatient, and specialty pharmacy teams to ensure safe and seamless transitions of care
• Partner with providers in clinic to perform proactive chart review, prepare and validate complex chemotherapy regimens, coordinate planned admissions, provide patient education and drug information support, and participate in pharmacist consult services to optimize therapy across the continuum of care
• Precept learners and conduct and lead research while contributing to a growing, world class oncology program

Qualifications

• PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy
• If hired for a specialty area where specialty residency training is available, then specialty residency is required
• Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire
• Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring
• For individuals required to administer immunizations: current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire
• For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or American Red Cross or certification obtained within 1 year of hire

Monday through Friday, from 8:00am to 4:00pm.

Pay

Minimum Annual Salary: $101,900.00 Maximum Annual Salary: $155,390.00
