Pharmacist - Medication Management - Rheumatology (SWWI)

Responsibilities

• Document and complete billing processes for pharmacist activities.
• Collaborate in protocol-driven practices and/or collaborative practice agreements as applicable.
• Contribute to quality improvement and resource management projects.
• Support research initiatives within the department and clinics.
• Participate in precepting activities for students and residents.
• Contribute to continuous improvement and patient safety efforts.
• Aid in other departmental projects and initiatives.
• Collaborate with Mayo Clinic Specialty Pharmacy, Infusion Therapy Center, and insurance prior authorization teams.

Qualifications

• Wisconsin Pharmacist License: Must be licensed to practice pharmacy in Wisconsin.
• Minnnesota Pharmacist License: Must be licensed to practice pharmacy in Minnesota.
• Educational Requirements: Must have a Doctor of Pharmacy (PharmD) degree or a Bachelor of Science in Pharmacy with at least 10 years of relevant experience if holding a Bachelor’s degree.
• Medication Therapy Management (MTM): If limited experience in Medication Management Ambulatory Care, completion of an accredited MTM certificate program is required within 12 months of hire.
• Mnnesota Health Care Programs (MHCP): Must be registered or eligible to be a registered MMS/MTM provider through Minnesota Health Care Programs (MHCP).
