Pharmacist - Kidney Transplant - Hybrid

About the role

Maintain the quality and support of highly immunocompromised patients requiring high level of specialized services.

Manage nephrology consults, pre-transplant infusions for highly sensitized patients, post-transplant infusion patients, and cover in-patient services including managing non-formulary requests.

Enter therapy plans, conduct chart reviews, compose letters of medical necessity, and participate in peer-to-peer reviews for denied insurance claims.

Participate in in-patient rounding which includes discharge teachings, medication reconciliation, ordering and coordinating last minute medication changes prior to discharge.

Responsibilities

  • Manage nephrology consults

Requirements

  • Current pharmacist licensure in the State of California
  • BLS certification as outlined in the Pharmacist Responsibilities in Cardiopulmonary Resuscitation (CPR) and Code Blue Procedure: Pharmacy Policy and Procedure

Qualifications

  • Pharm.D. or equivalent degree from an accredited school of pharmacy
  • Completion of a PGY1 and/or PGY2 residency or equivalent practice experience preferred

Skills

  • Comprehensive knowledge of medical terminology as related to drug usage
  • Comprehensive knowledge of common disease states and pathologies
  • Able to comprehend patient medical management plans
  • Able to assess and apply current pharmacological and biopharmaceutical principles for the selection and use of drug products in a clinical setting
  • Able to extract pertinent information from the patient or other sources and relate it to the patient care plan
  • Possess the verbal and written communication skills required to fulfill the pharmacist’s responsibilities
  • Able to teach and serve as preceptor for training programs
  • Able to prepare drugs needed for immediate administration
  • Able to provide recommendations and monitor the use of drugs
  • Possess patient-centered communication skills
