Pay
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position: $72.290 - $93.280 - $104.470 hourly. The actual pay rate and pay grade will depend on factors such as years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, and employer business practices.
Responsibilities
- Provide professional, clinical, and technical pharmaceutical care supporting Sharp HealthCare’s mission, values, and philosophy.
- Verification and review of medication orders:
- Review all medication orders for appropriate dose, duration, frequency, dosage form, and indication.
- Monitor therapy for potential or actual drug interactions (e.g., drug-drug, drug-disease, drug-allergy).
- Ensure compliance with inpatient drug formulary.
- Provide patient counseling on medication use, including first-dose teaching, food-drug interaction counseling, discharge counseling, and medication classes.
- Participate in medication reconciliation at admission and discharge, including documentation.
- Medication Therapy Monitoring:
- Ensure drug regimens align with Sharp medication use guidelines (e.g., antimicrobial use, IV to oral, renal dosing, serum drug concentration protocols).
- Adjust regimens based on approved protocols and interdisciplinary team consultation.
- Conduct daily profile reviews for each patient, assessing:
- Age, sex, height, weight, renal/hepatic function.
- Pregnancy and lactation status.
- Appropriateness of dose, route, rate, timing, frequency, indication, and therapeutic duplication.
- Available laboratory values and microbiology results.
- Appropriateness of antimicrobial therapy regimens, including planned stop dates.
- Potential drug-drug, disease-drug, food-drug interactions, or contraindications.
- Adherence to Sharp Pharmacy and Therapeutics Committee-approved guidelines/protocols.
- Targeted or therapeutic drug monitoring and pharmacokinetics, adjusting regimens as necessary.
- Review documented adverse drug reactions/allergies and report new occurrences.
- Assess disease progression and resolution of symptoms (e.g., pain).
- Communicate necessary information to healthcare providers, patients, and caregivers to encourage health promotion and prevention.
- Assess patient progression toward medication treatment goals.
- Demonstrate knowledge and competency in chemotherapy dosing, monitoring, and established guidelines (where applicable).
- Clinical Intervention Program:
- Participate in the clinical intervention program through issue identification, evidence-based recommendations, and follow-through with prescribers.
- Complete and document interventions consistent with department/shift averages.
- Communicate with nurses and physicians about therapeutic issues or problems in a timely manner.
- Follow up on formulary compliance issues and escalate to management as needed.
- Document interventions, patient contact, and multidisciplinary communication per Sharp policies.
- Address patient-specific drug information questions promptly and professionally.
- Pharmacy Operations:
- Prepare and dispense medications accurately and timely, adhering to standards of practice, legal/regulatory requirements, and hospital policies.
- Ensure controlled substances are dispensed accurately and maintain integrity of record-keeping.
- Assist with narcotic inventories as required.
- Demonstrate proficiency with automation and technology (EMR, Automated Dispensing Cabinet/Pyxis, IV Workflow Management/DoseEdge, physician order entry).
- Demonstrate knowledge of age-specific medication delivery systems (e.g., NICU admixtures, pediatric oral syringes, adult TPN/TNA solutions).
- Demonstrate competency in IV admixture/aseptic technique, including USP 797, 800, and 795 standards.
- Participate in Code-Blue emergencies (where applicable), mixing and labeling medications as needed.
- Understand investigational drug protocols and procedures for research studies (where applicable).
- Participate in cost management initiatives (e.g., reducing waste, managing inventory, using short-dated items).
- Oversee pharmacy operations and provide direction to Pharmacist Residents, Interns, Technicians, and non-licensed personnel.
- Safe Medication Practice, Regulatory, and Quality Improvement:
- Participate in medication safety event (MSE) monitoring programs, including interventions and RL submissions.
- Perform initial analysis of MSEs and assist in identifying causative factors.
- Conduct non-voluntary monitoring (e.g., trigger reports, harm monitors, alerting orders).
- Participate in medication use evaluations (MUEs) as assigned.
- Complete monthly nursing unit/drug storage area inspections, document discrepancies, and recommend improvements.
- Ensure security of medication storage and compliance with Medication Management (MM) standards.
- Monitor compliance with hospital, health-system, and department policies, as well as state/federal laws and regulatory agency standards.
- Participate in assigned department, hospital, and health-system committees or projects.
- Communication and Teamwork:
- Customer Service/Interdepartmental Relations:
- Answer phones in a timely, friendly manner.

- Operational Services:
- Communicate goals and instructions clearly to staff.
- Provide relevant handoffs between shifts (written/oral).

- Apply appropriate policies/procedures, including Chain of Command escalation for complex situations.
- Evaluate workflow issues and direct daily workload for efficiency.
- Attend and participate in staff meetings or review communications.
- Read work-related emails on assigned days and respond professionally.
- Customer Service/Interdepartmental Relations:
- Training and Mentoring:
- Engage in personal continuing education and professional development.
- Orient and train new pharmacy staff as assigned.
- Provide direction and precept Pharmacist Residents and Interns as assigned.
Requirements
- Education pursuant to licensure requirement.
- California Pharmacist License (CA State Board of Pharmacy) – REQUIRED.
- ACLS Certification (Advanced Cardiac Life Support) – American Heart Association – REQUIRED.
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) – American Heart Association – REQUIRED.
- AHA BLS and ACLS are required within 90 days of hire if not already certified.
Preferred Qualifications
- 1 year of experience in hospital pharmacy practice.
- Completion of a PGY1 Pharmacy Residency.
