Pharmacist - Full Time

The Pharmacist provides comprehensive pharmaceutical care through integrated drug distribution and clinical services, ensuring the safe and effective utilization of medications. This role is responsible for the dispensing and monitoring of medication therapy in accordance with all applicable regulations and standards.

Responsibilities

• Complies with and enforces all state and federal regulations and HMH and site-specific policies and procedures relating to all aspects of pharmaceutical care and medication management.
• Reviews and verifies all medications prior to dispensing from the pharmacy department, including checks for allergies, polypharmacy, drug-drug interactions, drug-food interactions, and clinical appropriateness. Communicates with providers as necessary.
• Oversees the dispensing, chain of custody, and accountability of controlled dangerous substances.
• Handles investigational drugs appropriately at applicable sites.
• Adheres to USP 795, 797, and 800 standards for sterile and nonsterile compounding and hazardous drugs in accordance with current regulatory standards.
• Oversees all activities of ancillary personnel (Pharmacy Technicians and Assistant/Couriers).
• Employs principles of safe medication best practices as recommended by the Institute for Safe Medication Practices, Joint Commission, and outlined in HMH guidelines, policies, and procedures.
• Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (e.g., age, culture, language, hearing and/or visually impaired). Communicates with patients, parents, and/or primary caregivers at their level.
• Provides pharmacological information by researching and answering questions and requests from other healthcare professionals.
• Actively participates in pharmacy clinical initiatives, including renal dose adjustments, IV to PO conversions, pharmacokinetic dosing, antibiotic stewardship, and opioid stewardship. Available for patient counseling upon request.
• Trains pharmacy staff and residents in all areas of pharmacy practice, including the location of medication management resources and guidelines. Educates pharmacy students and interns while on rotation.
• Completes network and departmental competencies, and licensing and certification requirements.
• Performs or oversees assigned unit inspections to assure adherence to medication management standards, including appropriate storage and security of medications.
• Performs other duties and/or projects as assigned.

Requirements

• Graduate of an ACPE accredited school of pharmacy, or equivalent, with a minimum of a BS degree in Pharmacy.
• Registered Pharmacist License.

Qualifications

• 1-2 years of hospital pharmacy experience (preferred).
• Knowledge of principles of USP 795, 797, and 800 (preferred).
• EPIC experience (preferred).

Pay

The minimum rate of pay for this role is $148,262.40 annually. The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Posted rates may vary based on labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding.

Full-time and part-time (20+ hours/week) positions are eligible for a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
