About the Role

Kindred Hospital Arlington is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. The Pharmacist works in collaboration with the Director of Pharmacy, compounding and dispensing medications and pharmaceutical supplies according to physician prescriptions. This role participates in Performance Improvement plans, consults on quality, regulatory, and risk issues, and supports clinical and medical staff. The Pharmacist interprets medication orders, compounds and dispenses pharmaceutical preparations, and supervises technical support staff while assisting with clinical, quality management, and education programs.

Responsibilities

• Participates in the maintenance and dissemination of drug information through in-service education, consultation with patients, and collaboration with hospital and medical staff.
• Assists other Pharmacists and Technicians in routine duties and provides professional and technical guidance to Pharmacy personnel.
• Exercises professional judgment in imparting therapeutic and drug information to patients and health professionals.
• Ensures competence in treating adult and geriatric patients, entering a minimum of 40 orders per four-hour shift.
• Generates required reports, including Pick I.V. and Production reports, and performs backup procedures.
• Assists Technicians in technical tasks, maintains aseptic techniques, and recycles products appropriately.
• Checks technician-prepared products prior to release and ensures proper maintenance of I.V. refrigerator temperature and hood cleaning logs.
• Fills orders accurately, prioritizes STAT, Now, and Waiting orders, and reviews patient profiles for safe drug therapy.
• Prevents therapeutic duplication, significant drug interactions, and avoidable adverse drug reactions (ADR) due to allergies.
• Ensures medication appropriateness for patient height, weight, age, renal function, and disease state.
• Provides Pharmacokinetics Dosing Service, including appropriate dosing, interpreting levels, modifying doses, and writing progress notes per protocol.
• Calculates fluid, electrolyte, and calorie needs, monitors formulation appropriateness, and clarifies orders with physicians.
• Stays current on new drugs, adds additives in the proper sequence, and ensures accurate syringe and vial counts.
• Labels containers accurately, completes packaging, and maintains security of the Pharmacy and controlled substances.
• Informs staff of pending issues, prioritizes workload, and keeps the Director informed of workload status.

Requirements

• Graduate of an accredited School of Pharmacy with a Pharmacist degree.
• Current state license in good standing.
• BCLS certification.
• Minimum of three (3) months of experience in pharmacy.
• Working knowledge of OSHA, JCAHO, HCFA, and state guidelines.
• Experience in facilitating teams and writing policies and procedures.

Skills

• Basic understanding of computer systems for processing prescription orders, with minimum word processing skills.
• Self-motivated, self-directed, and excellent organizational skills.
• Ability to handle multiple tasks simultaneously and organize meetings and inspections.
• Must read, write, and speak fluent English.
• Good and regular attendance.
