About the role
Reviews and processes medication orders ensuring appropriateness in terms of dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state.
- Ensures medication order appropriateness for age, weight, and disease state 100% of the time.
- Reviews medication orders for potential allergic reactions, drug interactions, therapeutic duplication, and drug incompatibilities 100% of the time.
- Verifies PRN medication orders contain indications and are documented 100% of the time.
- For medication orders outside the normal range, ensures the reason and rationale for use is appropriate and documented in the Pharmacy Intervention Record.
- Provides drug information and educational programs to healthcare professionals and patients.
- Consults with physicians, nurses, and other healthcare professionals on drug dosages, forms, availability, side effects, and incompatibilities.
- Advises patients and families on medication use, including names, descriptions, routes, dosages, durations, directions, side effects, interactions, contraindications, self-monitoring, storage, refills, and actions to take if doses are missed.
- Trains new pharmacy staff in compliance with training goals and objectives.
- Ensures compliance with CHOP Formulary System and reviews non-formulary medications for appropriateness or approval by clinical pharmacists.
- Supervises pharmacy technicians and interns to ensure accurate preparation and distribution of pharmacy products.
- Prepares and distributes sterile and non-sterile products accurately and safely according to USP guidelines and department standards.
- Ensures medication compatibility and stability, and properly repackages doses with quality control records.
- Documents transactions accurately and securely, ensuring no discrepancies in logs.
- Destruction of returned controlled substance continuous medication infusions is managed.
- Participates in hospital and departmental Continuous Quality Improvement activities.
- Utilizes Lean Principles to analyze and resolve problems systematically.
- Works overtime to assist coworkers in Lean Projects (RPI/5S).
- Provides feedback and support to sustain successful measures.

Requirements
- Bachelor's Degree in Pharmacy - Required
- Doctorate in Pharmacy - Preferred
Qualifications
- Knowledge of pharmacy information systems, automated dispensing technology, and personal computers preferred.
- Effective interpersonal and communication skills.
- Demonstrated knowledge and skills necessary to provide care to neonate, child, and adolescent preferred.
- Prior Emergency Department experience preferred.
- Prior Peds Pharmacy experience preferred.
- Licensed Pharmacist (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required

Pay
SALARY RANGE: $55.32 - $73.29 Hourly
Schedule
This position works evening shift (3:00 PM-11:30 PM), along with every third weekend.
