About the role

The clinical pharmacist will be a unit-based pharmacist that will verify orders and provide clinical services for the assigned area(s). They will serve as a drug information resource for nurses, physicians, patients, and families.

Responsibilities

  • All general responsibilities of a Staff Pharmacist
  • Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals
  • Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions
  • Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution
  • Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management
  • Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice based on MUE findings

Requirements

  • Pharm. D. or BS in Pharmacy
  • Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
  • Pharmacy license
  • BLS within 30 days of hire (ACLS and/or PALS if code attendance required)
  • Board Certification in Pharmacotherapy (BCPS)

Pay

$36.95 - $57.27 per hour. Individual wages are determined based on qualifications and experience.
