Pharmacist Clinical - Hospital Pharmacy - Per Diem

Elliot Hospital Inpatient Pharmacy is comprised of pharmacists and technicians working collaboratively to provide safe, timely, and effective medication therapy to patients. The department operates a decentralized model for medication distribution using Omnicell automation and clinical pharmacist support on nursing units. Elliot promotes career advancement through an ASHP-accredited PGY-1 Pharmacy Residency Program, a technician career ladder, and reimbursement for professional certifications.

About the Role

The Clinical Pharmacist operates in both central distribution and decentralized nursing unit settings, leveraging strong clinical aptitude and a team-first attitude to positively affect patient outcomes in a multidisciplinary healthcare environment.

Responsibilities

• Medication Order Processing: Prompt and accurate processing of computer physician order entry (CPOE) via the electronic health system (EHR).
• Clinical Pharmacy:
  • Monitors drug therapy to prevent contraindications, duplicate therapy, drug interactions, and allergic reactions.
  • Recommends appropriate medication doses and adjustments based on patient conditions.
  • Responds to drug information requests with accurate, clinically relevant, and timely information.
• Collaboration with Healthcare Team: Participates in multidisciplinary team rounding with physicians, physician assistants, nurse practitioners, nursing, case management, and other disciplines.
• Patient Interactions:
  • Interacts with patients and caregivers to collect accurate medication histories and provide education when appropriate.
  • Responds to critical patient situations (e.g., traumas, codes) to provide pharmacy support as part of the medical response team.
• Education: Provides training and education to pharmacists, nurses, providers, students, and residents as applicable.
• Development:
  • Actively engages in personal professional development and growth to support clinical care of patients.
  • Completes education and training beyond licensure requirements.
  • Works to improve work and departmental performance.
• Pharmacy Operations:
  • Understands and adheres to hospital formulary whenever possible and recommends therapeutic interchange as necessary.
  • Participates in medication utilization or other assessments of clinical, cost-effective use of medications.
  • Oversees technician duties, ensuring accuracy and organizing workflows to meet patient needs and improve care.
  • Provides leadership and direction in oversight responsibility with technicians.

Requirements

• PharmD or BS in Pharmacy required.
• Current New Hampshire Pharmacist license to practice Pharmacy required.
• Board of Pharmacy Specialist (BPS) certification strongly preferred.
• Residency preferred.
• Basic Life Support Certification preferred but not required.
• One (1) year of previous pharmacy experience preferred.
• Epic Willow experience preferred.
• Knowledge of New Hampshire drug laws preferred.
