Pharmacist, Clinical for Home Infusion, Monday-Friday

Job Description Growing home infusion company seeking a Clinical Pharmacist to be part of our team. Previous home infusion experience preferred. IV certification preferred but not required. Experience and strong knowledge base in sterile compounding of intravenous (IV) medication admixtures including total parenteral nutrition formulations with adherence to USP 797 guidelines. Facilitates new patient admissions through collaboration with nursing and intake staff. Receives, verifies, reviews and documents all new patient orders. Establishes parameters for monitoring the safety and efficacy of drug therapy. Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information. Communicates the results of therapeutic drug monitoring activities to the physician, nurse and other appropriate persons. Completes and maintains appropriate documentation related to medication profiles, progress notes, assessments, care plans and all other clinical activities, in accordance with applicable state laws, regulatory agencies, Company policy and procedure, and accreditation standards. Checks compounded medications to ready for delivery. Programs and troubleshoots infusion control devices as indicated. Bachelor’s degree in pharmacy and clinical experience is acceptable in lieu of PharmD. Compensation based on experience.
