Responsibilities

• Coordinate and provide pharmaceutical care for patients, collaborating with an interprofessional team to optimize safe, effective medication use.
• Review and verify orders, manage drug therapy protocols, educate staff and patients, and support quality and regulatory standards.
• Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
• Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Document interventions and outcomes for drug therapy quality improvement.
• Verify medication orders for accuracy and completeness prior to entry.
• Check filled medications prepared by technicians.
• Maintain proficiency with pharmacy automated systems and software.
• Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
• Participate in performance improvement and required continuing education.

Requirements

• Doctorate degree in Pharmacy.
• Current license with the state Board of Pharmacy.
• BLS (Basic Life Support) certification required.
• ACLS (Advanced Cardiovascular Life Support) preferred.
