Pharmacist - Case and Disease Management (CMDM) - Remote

For our client, we are seeking a Pharmacist - Case and Disease Management (CMDM) - Remote to join the team of a leader in the Finance & Insurance space. This role focuses on supporting high-risk members through case and disease management, with an emphasis on identifying care gaps and coordinating clinically appropriate interventions. You will collaborate with a multidisciplinary team and engage directly with members, caregivers, physicians, and other pharmacists to improve care outcomes. The position also calls for strong clinical judgment, research skills, and thoughtful decision-making in a remote, flexible work environment. Location: Remote - US based candidates only, no visa sponsorship available Compensation: $118,400 – $196,800 annually Responsibilities Review gaps in care for high-risk patients as part of a multi-disciplinary team Conduct research to evaluate patient situations and provide clinical recommendations Intervene on gaps of care by contacting patients, caregivers, physicians, and pharmacists Utilize high levels of clinical judgment and discretion in care management Perform additional duties as assigned Qualifications Bachelor's Degree in Pharmacy required; Doctorate in Pharmacy preferred 5 - 7 years of experience in Pharmacy 3 - 5 years in Utilization Management as a Pharmacist 3 - 5 years in Medical Therapy Management as a Pharmacist Registered Pharmacist certification required Benefits Opportunity to work in a collaborative, multi-disciplinary team environment Engagement with high-risk patients to make significant impacts on their care Development of clinical research and evaluation skills Utilization of strong judgment and clinical expertise in patient care Potential for flexible office-based work with limited travel requirements Our client is an equal opportunity employer.
