About the Role
Reston Hospital Center, part of HCA Healthcare, offers an exciting opportunity for a Pharmacist. The role involves assessing physician orders for accuracy, drug interactions, and appropriate dosing, as well as documenting interventions and communicating with healthcare teams. The pharmacist will counsel patients on medication use, particularly for special care nursery, pediatric, and patients discharged on seven or more medications.
Responsibilities
- Assess physician orders for accuracy, drug allergies, drug-drug interactions, and appropriate dosing based on patient age/weight.
- Document all interventions and communicate effectively with healthcare workers.
- Counsel patients on medication use, including discharge counseling for special care nursery, pediatric, and patients on seven or more medications.
- Monitor drugs with narrow therapeutic ranges, problem-prone medications, and high-use medications to ensure positive patient outcomes.
- Review antibiotic therapy for appropriateness based on culture and sensitivity reports, and recommend cost-effective alternatives.
- Ensure proper storage and handling of medications by completing unit inspections monthly.
- Assist in teaching programs for nursing personnel and other healthcare providers.
- Provide drug information to patients, nurses, physicians, and other healthcare providers.
- Direct the work of technicians, including IV admixture, preparation of intensive care nursery medications, TPN, chemotherapy, and unit dose prepacking.
- Attend codes and provide pharmaceutical expertise.
- Monitor medications for patients 65 years or older with decreased creatinine clearance.
- Review and prepare medications for special care nursery and pediatric patients per policy.
- Check exchange medication carts prior to delivery.
- Fill and check employee prescriptions, ensuring all documentation is completed and counseling employees.
- Reorder stock as needed and make rounds at least twice per shift, ensuring rounds occur hourly.
- Conduct narcotic inventory 100% of the time as per protocol.
Requirements
- Graduate of a college of pharmacy with a BS or Pharm.D accredited by the American Council on Pharmacy Education.
- Licensed Pharmacist in the Commonwealth of Virginia.
- BLS Certification.
- Previous experience as a Pharmacist (hospital pharmacy experience preferred).
