Job Type Full-time Description We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Job Type: Full-Time, 40 hours per week, Mon-Fri 8:00am-5:30pm CST Job Location: Lenexa, KS Requirements B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduateActive registered pharmacy license in the state of Kansas and additional states as required without restrictions1 year of hospital and home infusion therapy experience preferredTwo years of recent clinical pharmacy experience preferredThorough knowledge of infusion therapies preferredRecent inventory management experience preferredAccuracy in mathematical calculations including pharmacokinetic dosing preferredKnowledge of CPR+ software preferred Job Responsibilities, Included But Not Limited To Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply managementCollect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completenessMonitor the patient’s progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. Most patient contact will be on the phone, and, on occasion, you will meet them in our infusion suiteDirectly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging, and labeling of compounded sterile productAssist in training and orientation of new employees, performance improvement activities and educational programsAdditional licensure may be required in multi-state service areasGo in to IV room and mix medications if neededOther duties assigned Physical Demands The physical requirements listed here indicate what an employee must meet to effectively perform this role's essential functions.
