Responsibilities

  • Work with Physicians, Nurses, and patients to manage infusion therapies.
  • Prepare and dispense intravenous medications, ensuring proper dosages, sterility, and safety protocols.
  • Formulate and adjust Total Parenteral Nutrition (TPN) solutions based on individual patient needs.
  • Maintain proper blood levels within therapeutic range to avoid toxicity or suboptimal effects.
  • Program and set up ambulatory infusion pumps for home-based therapy, ensuring proper flow rates, alarm settings, and patient safety.
  • Accurately enter and verify prescriptions, ensuring correct dosages, drug interactions, and patient history.
  • Assess medication needs and history for new patient cases, providing consultation and patient counseling.
  • Collaborate with healthcare teams to ensure optimal patient care and medication management.
  • Adhere to all pharmacy laws, regulations, and best practices for patient safety and medication accuracy.

Requirements

  • Registered Pharmacy License.
  • At least 1 year of Home Infusion or Specialty Pharmacy experience.
  • IV Dosing & Sterile Compounding - Experience with preparing and dispensing intravenous medications.
  • TPN Formulations - Experience formulating and adjusting TPN solutions.
  • Vancomycin Monitoring - Experience monitoring Vancomycin therapy and adjusting dosing.
  • Ambulatory Infusion Pump Programming - Experience programming and setting up ambulatory infusion pumps.
  • Patient Education - Experience providing patient education on pump usage, troubleshooting, and emergency procedures.
  • Prescription Entry & Verification - Experience accurately entering and verifying prescriptions.
  • New Patient Intake & Consultation - Experience processing new patient cases, assessing medication needs, and providing consultation.
  • Quality Control & Compliance - Experience adhering to pharmacy laws, regulations, and best practices for patient safety and medication accuracy.

Qualifications

Candidates must have strong phone and communication skills to counsel patients.
