Responsibilities
- Conduct patient interviews to obtain accurate and complete medication histories during admission or transition of care
- Review and verify physician medication orders for appropriateness, dosage, route of administration, frequency, and potential drug interactions
- Ensure medication orders comply with hospital protocols, clinical guidelines, and patient-specific factors (e.g., renal function, allergies, weight)
- Prepare and dispense medications accurately, including IV admixtures, unit-dose medications, and controlled substances, following all safety and sterility protocols
- Monitor medication therapies and provide clinical recommendations to optimize therapeutic outcomes and minimize adverse drug events
- Collaborate with physicians, nurses, and other healthcare professionals to develop and adjust patient-specific medication plans
- Cook up daily operations within the pharmacy to ensure efficient medication distribution and inventory management
- Deliver pharmaceutical care to a wide range of inpatient units, including – General Medicine/Surgery , Intensive Care Units (ICU) , Obstetrics & Gynecology , Pediatrics , Emergency Department , Oncology , Perioperative Services , Geriatrics and other departments
Qualifications
- Current state Registered Pharmacist License
- Experience in hospital pharmacy operations preferred
- B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred
- BLS certification required within 6 months of hire and must be maintained
