Pharmacist
About the role
This position offers a licensed pharmacist the opportunity to support the Phoenix VA Health Care System by delivering high-quality inpatient and outpatient pharmacy services to Veterans. You will play a key role in ensuring safe, effective, and appropriate medication therapy while collaborating closely with an interdisciplinary healthcare team in a mission-driven environment.
Responsibilities
- Dispense medications prescribed by physicians and other authorized healthcare providers in accordance with established policies and regulations.
- Review and verify medication orders for accuracy, appropriate dosing, and clinical appropriateness based on patient-specific factors.
- Evaluate medication orders for potential drug–drug and drug–food interactions, allergies, and contraindications, and intervene when necessary.
- Identify and resolve therapeutic issues, including duplicate therapies, incorrect dosing, or inappropriate frequency, to optimize patient outcomes.
- Provide clear and thorough medication counseling to patients, including usage instructions, expected benefits, potential side effects, and adverse reactions.
- Maintain accurate, timely, and complete documentation in the electronic medical record system, including VistA and CPRS.
- Contribute to quality improvement initiatives by participating in data collection, reporting patient outcomes, and supporting clinical performance measures.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement safe and effective medication therapy plans.
- Adhere to all applicable pharmacy laws, regulations, and facility policies to ensure compliance and patient safety.
Essential Skills
- BS in Pharmacy or Doctor of Pharmacy (PharmD) degree required.
- Active, unrestricted pharmacist license in any U.S. state, territory, or the District of Columbia.
- At least 2 years of relevant pharmacist experience within the last 36 months.
- Experience in inpatient and/or outpatient pharmacy practice.
- Ability to accurately review and verify medication orders for dosing, appropriateness, interactions, and contraindications.
- Strong clinical knowledge and ability to interpret and manage medication therapies for diverse patient populations.
- Proficiency with electronic medical record documentation and pharmacy workflows.
- Ability to communicate effectively in English, both written and verbal, with patients and healthcare team members.
- Strong attention to detail, organization, and commitment to patient safety.
Additional Skills & Qualifications
- Familiarity with VA systems, particularly VistA and CPRS, is highly desirable.
- Experience working in a hospital or health system pharmacy setting, including inpatient and/or outpatient environments.
- Demonstrated ability to provide patient-centered medication counseling and education.
- Strong interpersonal skills and ability to work collaboratively within a multidisciplinary, mission-driven healthcare team.
- Commitment to continuous quality improvement and participation in initiatives aimed at enhancing patient outcomes.
- Interest in long-term engagement, with potential contract stability of up to five years.
- Motivation to serve and make a direct, positive impact on Veteran care.
Work Environment
The role is based in a combined inpatient and outpatient dispensing pharmacy environment within a large, collaborative healthcare system. You will work with electronic medical record systems, including VistA and CPRS, and engage with both clinical and operational pharmacy workflows. The setting emphasizes teamwork, mission-driven care for Veterans, and exposure to a wide range of inpatient and outpatient pharmacy practices. The environment is professional, structured, and focused on patient safety, medication security, and high-quality clinical service delivery.

Pay And Benefits
The pay range for this position is $60.00 - $60.00/hr.
