Position Overview

We are seeking a patient-focused Pharmacist with strong counseling experience to join a fast-paced, clinical environment. This role is highly interactive, requiring extensive time on the phone providing patient education and support.

Key Responsibilities

  • Counsel patients by phone on medication therapy, usage, side effects, and adherence
  • Conduct comprehensive clinical reviews, including drug utilization, interaction screening, and therapy optimization
  • Provide education on new drug therapies and support patient understanding of treatment plans
  • Document pharmacotherapy reviews and patient interactions in clinical records
  • Cook up care and clinical duties
  • Coordinate care with prescribing clinicians and healthcare teams to ensure continuity of care
  • Serve as a liaison between patients, caregivers, nurses, and providers
  • Provide subcutaneous injection teaching and coordinate additional clinical support as needed
  • Serve as a liaison between patients, caregivers, nurses, and providers
  • Support intake of new patient referrals and ensure appropriate documentation
  • Follow REMS program requirements and standard operating procedures
  • Participate in occasional on-call coverage as assigned
  • Pharmacy Operations
  • Verify prescription accuracy and ensure compliance with all applicable regulations
  • Perform quality assurance checks on all prescriptions prior to dispensing
  • Process and verify prescriptions in a timely manner
  • Oversee and support pharmacy technicians
  • Assist with pharmacy claim adjudication processes
  • Ensure compliance with federal, state, and board of pharmacy laws
  • Maintain adherence to internal policies and procedures
  • Monitor hazardous material handling and pharmacy safety standards
  • Support adverse event and product quality reporting as required

Qualifications

  • Active Pharmacist license in good standing
  • Minimum of 2 years of experience in patient counseling and prescription verification
  • Strong communication skills and comfort with high-volume phone interactions
  • Experience in retail or independent pharmacy settings preferred
  • Hospital experience considered, though patient counseling experience is essential
  • Ability to multitask and work efficiently in a fast-paced environment
