PH RN Case Manager – CMS

Responsibilities

• Provide comprehensive support to families of children with special healthcare needs under general supervision, following established policies, procedures, and protocols.
• Develop and manage individualized care plans.
• Coordinate ongoing care and facilitate transition planning starting at age 12.
• Actively engage and retain families in the program while promoting CMS services throughout the district.
• Build and maintain relationships with community partners and participate in outreach activities.
• Manage enrollments, conduct six-month and annual reviews, and ensure continuity of care.
• Review medical records, approve billing and documentation for other nurses.
• Assist with phone inquiries as needed.

Requirements

• Associate degree in nursing from an accredited college or university.
• Completion of a PHN course within one (1) year of hire.
• Current Georgia Registered Professional Nurse license.
• CPR certification.

Qualifications

• Experience working with infants or young children (preferred).
• Bachelor’s degree in nursing (BSN) (preferred).
• One year minimum experience as a registered professional nurse (preferred).
• Bilingual proficiency in Spanish (preferred).
