Responsibilities

• Assist with bathing, toileting (including use of bedpan, urinal, commode, or toilet), dressing, and undressing
• Support mobility and ambulating, including use of walker, cane, and crutches
• Perform transfers, such as bed to chair or wheelchair and reverse
• Change consumer’s bed and launder bed linens and personal clothing
• Provide skin care (excluding wound care)
• Care for eyeglasses and hearing aids
• Perform light cleaning in essential areas of the home during personal care activities
• Prepare and serve meals
• Promptly report changes in the consumer’s condition to the R.N. Case Manager
• Complete all time sheets and documentation necessary to maintain program services thoroughly, accurately, and timely
• Maintain confidentiality of consumer’s information

Requirements

• Familiarity with the philosophy and concepts of independent living
• Ability to understand and carry out consumer instructions
• Good communication skills and ability to relate well to others
• Good past employment references
• Physical strength sufficient to perform essential functions of the job
• Ability to regularly sit, stand, walk, use hands and fingers, handle, and feel; reach with hands and arms; bend, squat, twist, and talk or hear
• Ability to occasionally lift and/or move individuals using transferring techniques or assistive technology/equipment
• Reliable vehicle or alternate means of transportation to worksite
• Completion of interview and training conducted by PAS staff
• Satisfactory criminal background results (based on relevance to position and in compliance with state and federal regulations for personal care workers)
