Responsibilities

• Lead and support the Neighborhood Care Team
• Administer medications and treatments accurately
• Assess and monitor resident health; communicate changes promptly
• Delegate daily assignments and ensure quality, person-centered care
• Mentor CNAs, PCAs, and team members
• Collaborate on individualized care plans
• Ensure regulatory and HIPAA compliance
• Work a flexible schedule, including weekends/holidays

Qualifications

• Graduate of an approved LPN program (RN candidates welcome)
• Active, unencumbered PA LPN license
• Prior experience in Gerontology or Long Term Care preferred
