Personal Care Attendant- PACE

About the role

The PACE Personal Care Assistant (PCA) is a member of the PACE Interdisciplinary Team (IDT). The PCA provides personal care services in accordance with individual care plans for PACE participants and performs other PACE Center tasks as assigned by management. The position is located at the PACE of Cincinnati center (4850 Smith Rd, Cincinnati, OH 45212) and requires driving within the Greater Cincinnati area.

Responsibilities

• Provides personal care services per participant care plans – peri‑care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
• Assists with food service delivery and monitoring during meal service.
• Implements interventions as directed by IDT disciplines, including mechanical assistance for transfers.
• Accompanies participants to outside medical appointments as assigned.
• Monitors participants for health or psychosocial changes and communicates pertinent changes to the IDT or management.
• Attends IDT and morning meetings as assigned.
• Completes PCA documentation in the PACE EHR per federal regulation and organizational policy.
• Supports the PACE QI program and participates in QI activities as assigned.

Working conditions include frequent bending, occasional climbing, consistent concentration, consistent hearing (conversation and other sounds), consistent interpersonal communication, occasional kneeling, consistent lifting

Requirements

• High School Diploma or GED (Preferred)
• Up to 1 year experience working with a frail or elderly population (Preferred)
• Must have reliable transportation
• Basic Life Support (BLS) Upon Hire Required
• Acceptable driving record
