Job Responsibilities
- Provides personal care services in accordance with individual participant care plans – including peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
- Affords assistance with food service delivery and monitoring during PACE Center meal service.
- Implements interventions as directed by IDT disciplines, including mechanical assistance for participant transfers.
- Accompanies participants to outside medical appointments as assigned by PACE Center management personnel.
- Maintains vigilance over PACE participants for changes in their health or psychosocial status and promptly communicates pertinent changes in participant status to the IDT or PACE Center management personnel.
- Attends IDT and morning meetings as assigned by PACE Center management personnel.
- Completes PCA documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy.
- Supports the PACE QI program and participates in PACE QI activities as assigned by PACE Center management personnel.
Job Requirements
- High School Diploma or GED (Preferred)
- Up to 1 year Experience working with a frail or elderly population (Preferred)
- Must have reliable transportation
- Basic Life Support (BLS) Upon Hire Required and an acceptable driving record. Upon Hire Required Basic Life Support (BLS) Required

Job Overview
The PACE Personal Care Assistant (PCA) is a member of the PACE Interdisciplinary Team (IDT). The PCA is responsible for providing personal care services in accordance with individual care plans for PACE participants. The PCA is responsible for providing personal care services and other PACE Center tasks as assigned by PACE Center management personnel.
