Responsibilities

• Perform and assist with personal care ADLs/IADLs for participants in the PACE across all platforms of delivery of care, such as center, in‑home, or virtual settings.
• Be able to perform the job duties of a PCA I if assigned.
• Support the PCA III in gathering and reporting information related to care coordination, participants’ ability to engage, maintain/improve ability, foster independence, maintain function, improve socialization, or satisfaction.
• Communicate and collaborate with IDT members or other PACE staff on the goals of the plan of care, focusing on needs (e.g., participant supplies or tools), health status, physical, and mental well‑being.

Requirements

• Certified Nursing Aide education and training.
• Nurse Assistant Certificate issued by the State of California, Department of Health and Human Services‑Aid and Technician Certification.
• High school diploma required; some college education/credits preferred.
• California Food Handler Certificate of Achievement preferred upon employment (must be obtained within 30 days of hire).
• Prior experience working with physically or mentally impaired older people strongly preferred.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location, preferred.
• Experience working with the elderly patient population.
• Valid BLS certification or higher, following American Heart Association (AHA) or American Red Cross guidelines.

Pay

$25.00 - $29.32 hourly. Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change. AltaMed is committed to ensuring a fair and competitive compensation package that reflects the candidate's value and the role's strategic importance within the organization.
