Personal Care Attendant - Full Time

About the role

Habitat Health is expanding its PACE model to meet the growing needs of aging populations. We are seeking a mission-driven team player to join our care teams.

Responsibilities

• Contribute to a center experience that Participants want to spend time in
• Care for Participants' personal hygiene and basic needs
• Assist with activities of daily living
• Respond to and fulfill Participant requests
• Perform and assist with assigned non-sterile procedures
• Prepare and plate meals
• Ensure Participant comfort and physical privacy
• Collect and record data
• Provide Participant instruction related to safety factors
• Perform light housekeeping tasks
• Occasionally provide homemaking services in the home
• Occasionally accompany Participants for outside appointments
• Observe Participants for changes in condition
• Report Participant changes of condition promptly

Requirements

• Completion of accredited vocational nursing assistant course, hospital, or comparable participant care experience
• Knowledge of PACE program
• Ability to work independently and manage changing priorities
• Effective interpersonal skills
• Able to work with individuals of diverse ethnic and cultural backgrounds
• Physical strength to perform essential functions
• A minimum of one year experience working with the frail or elderly

Qualifications

• High School diploma/GED
• Comparable experience in skills such as TPR, BP, bed bath, and ADL
• Knowledge of physical, mental, and social needs of frail older adults
• Able to manage changing priorities per participant needs
• Effective interpersonal skills to work with participants, caregivers, and staff
• Able to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds
• Able to be self-directed and able to work independently
• Requires physical strength to perform essential functions of the job
• A minimum of one year experience working with the frail or elderly

• Bilingual: Spanish (preferred)

Pay

The anticipated base salary range for this position is $22.00 to $26.00 / hour. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.

Onsite in Sacramento, CA.
