Personal Care Aide

Summary/Objective

Provides oversight and supervision to individuals with disabilities to maintain their health and welfare, assisting them with Activities of Daily Living.

Qualifications

• Certification as a Personal Care Aide in New York State preferred but not required - agency will provide training
• Knowledge of community resources and means of accessing these services
• Valid NYS drivers license/dependable vehicle
• Demonstrates basic reasoning and writing skills
• Six (6) months of experience providing personal care to individuals with disabilities required
• Basic understanding of medical terminology

Responsibilities

• Imparts agency philosophy of Equal Opportunity, Independence and Realization of Individual Potential to clients, families, program staff and community resources
• Maintains confidentiality of information relating to individuals and their families in accordance with agency policy
• Provides care in accordance with the Plan of Care and physicians orders; after supervising RN has approved the individual to provide said care
• Assists with implementation of behavior modification plans as needed
• Participates in case conferences upon request
• Communicates regularly with agency personnel to provide opportunities for exchange of information and discussion of concerns
• Performs other duties at the discretion of the Program Director or his/her designee

Other Job Specifics

• Directly responsible to Home & Community Support Services Supervisor and Supervising Registered Nurse
• A minimum of 6 hours of training is required per year for this position
• Adheres to the agencys policies and procedures as well as department guidelines
