PERM PHYSICAL THERAPIST - PT

Responsibilities

• Provide patient-centered physical therapy assessments and develop individualized treatment plans.
• Deliver evidence-based PT interventions to restore function and reduce pain.
• Document patient progress and maintain accurate, timely records in EHR.
• Collaborate with multidisciplinary team to coordinate care and referrals.
• Set and track performance targets using pipelines to monitor progress.
• Maintain compliance with regulatory standards, safety, and infection control.
• Educate patients and families on exercises, home programs, and self-management.
• Participate in professional development initiatives and pursue advancement opportunities.
• Demonstrate initiative to improve clinic processes and patient experience.
• Absorb and schedule patients to maximize throughput.
• Participate in on-site audits and quality improvement projects.

Requirements

• PT licensure in the operating jurisdiction with active state license.
• Bachelor’s or Doctorate in Physical Therapy from an accredited program.
• Minimum 1–2 years of outpatient PT with manual therapy and gait training.
• Proficiency with EHR documentation and standardized patient progress notes.
• Strong teamwork to coordinate multidisciplinary referrals and care plans.
• Knowledge of infection control, safety regulations, and regulatory compliance.
• Comfort with targets, pipelines, and performance metrics to drive throughput.
• Demonstrated initiative to improve clinic processes and patient experience.
