Requirements

• Valid NYS Nursing License
• Government ID, Social Security Card, Work Authorization
• 2 reference letters
• Recent physical examination results
• TB Screening (Quantiferon preferred)
• Rubella & Rubeola titer lab work

Pay

$75 to $100 per Visit
