Per Diem RN Care Coordinator

About the role

The Per Diem RN plays a crucial role in providing outstanding patient care and high-level customer service in various settings. They are part of a dedicated team committed to helping terminally ill patients and their families.

Responsibilities

• Achieve patient/family needs through the nursing process and coordination with the Interdisciplinary Group (IDG).
• Provide palliative and supportive care to the patient/family unit.
• Complete clinical admissions, explaining services, addressing patient/family concerns, and initiating the Plan of Care.
• Communicate with physicians and obtain medical records for referrals.
• Manage patient discharges efficiently, ensuring timely follow-ups and adherence to productivity expectations.
• Facilitate referrals to all service lines, working closely with referring hospitals, physicians, facilities, patients, families, and the general public.
• Participate in patient deaths, preparing bodies for transport, overseeing disposal of medications, and providing bereavement support.
• Supervise patient care given by other nursing team members, including LPNs, Hospice Aides, and contract nurses during emergencies.
• Perform other duties as assigned, including triage during emergencies.

Qualifications

• Current license as RN in the state where working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• One (1) year nursing experience; hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR system.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Effective communication in English (verbal and written).
• Flexible schedule to meet the needs of Hospice House patients.
• Ability to manage emotional stress of working with and caring for terminally ill patients and their families.
• Ability to physically access patient rooms, other areas of the Hospice House/Hospital/ALF, etc., and any home (navigate stairs and narrow spaces, tolerate heat and lack of air conditioning).
• Physical ability to perform essential functions and physical requirements (lifting patients and/or equipment, bending, pushing/pulling, kneeling).
