Per-Diem Registered Nurse (RN) Home Health

What You'll Be Doing

• Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors
• Develop and implement individualized care plans that are tailored to each patient's needs and goals
• Provide patient education and support to promote self-management and prevent complications
• Cookordinate care with other healthcare providers, including physicians, therapists, and social workers, to ensure continuity of care
• Administer medications, treatments, and procedures as ordered by the physician or authorized healthcare provider
• Document all patient care activities and communicate patient status and progress to the healthcare team
• Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction
• Maintain professional knowledge and skills by attending continuing education programs and staying up to date with the latest developments in the field
• Provide emotional support and compassionate care to patients and their families
• Adhere to all applicable laws, regulations, and ethical standards of nursing practice

Requirements

• A current and unrestricted RN license in the state of Florida
• Strong clinical skills and ability to perform evidenced based nursing practices
• One to three years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health
• Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare providers
• A physical exam and two-step TB test or chest X-ray (no older than 6 months)
• A yearly mandatory in-service
• Proficiency in using electronic health records and other healthcare technologies
• A valid FL driver's license and reliable transportation (Auto Insurance must also be presented)
• CPR certification required (Original Card)
• A level 2 criminal background check
