Per Diem - Pharmacist
About the role
Responsible for performing and documenting patient pharmacy clinical interventions, education, and counseling that pertains to medication management and safety. Interprets, evaluates and reviews of physician inpatient orders and outpatient prescriptions in order to prepare, calculate, compound, admix and dispense appropriate medication and dosages consistent with optimum patient care.
Responsibilities
- Engages in practices that promote and advance professional practice, education, patient safety, and collaboration with other health care practitioners.
- Interprets, evaluates and reviews of physician inpatient orders and outpatient prescriptions in order to prepare, calculate, compound, admix and dispense appropriate medication and dosages consistent with optimum patient care.
- Ensures the dispensation of the right medication, at the right dosage, for the right medical indication, at the right time, to the right patient.
- Provides direct patient care to neonate, pediatric, adolescent, young adult, adult and geriatric patients.
- Staffs a minimum of 4 shifts per month on weekends and/or weekdays to meet the needs of the department.
Requirements
Work requires the knowledge of theories, principles, and concepts acquired through completion of a PharmD (BS Pharmacy degree may be accepted as entry level for pharmacist position).
NJ Board of Pharmacy Licensure and registration required.
Pharmacy Residency and Board Certification in clinical pharmacy practice (e.g. Board Certification in Pharmacotherapy BCPS) Certificate is preferred.
New hires have one year from date of hire to receive Advanced Cardiac Life Support Certification (ACLS).

Pay
Pay transparency: St. Joseph’s Health provides a salary range to comply with New Jersey Law. The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc.” The salary range does not include incentives, differential pay or other forms of compensation.
