Per-diem Patient Assessment Coordinator

About the role

Kindred Hospital Dayton and Behavioral Health Unit (BHU) is a 97-bed hospital with 51 long-term acute care hospital (LTACH) beds and 46-bed behavioral health unit. The unit offers 24-hour supervised psychiatric treatment and crisis support for mental health issues.

Responsibilities

• Conduct patient assessments in accordance with company standards for appropriate patients for potential admission into the system.
• Work closely with patients and their family members to educate them on the benefits of Kindred Hospital.
• Aid in conducting tours of the hospital with families and referral sources as needed.
• Facilitate the timely admission of patients from the referral sources:
• Identify medical record information needed from referring facility;
• Complete the Pre-Admission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission;
• Facilitate and coordinate physician-to-physician communication as necessary;
• Identify specific patient needs for equipment, supplies and consult services.
• Maintain positive relationships with referral sources, sales teams, admission, managed care, clinical and operational teams.
• Coordinate transmission of clinical and benefits information of patients for possible admission into our ScionHealth Specialty Hospitals.
• Collect, organize, evaluate, and communicate pertinent clinical information with effective verbal and written skills.
• Possess strong attention to detail and commitment to following all processes set forth.
• Operate with a sense of urgency to produce quick results.
• Maintain all patient information in a manner that ensures confidentiality and meets all regulatory requirements.
• Meet with management staff, participate in marketing meetings and serve on various internal committees as assigned.
• Maintain a state-of-the-art knowledge of areas of responsibility and develop and follow a program of continuing education.
• Conduct job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.

Requirements

• Associate or Bachelor’s degree in healthcare related field preferred.
• Healthcare professional licensure preferred. License options include: Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker.
• Two (2) years clinical patient care/treatment experience in healthcare field preferred.
• Demonstrated understanding of Medicare, Medicaid, and other payers as related to LTACH benefits.

• Computer skills that include Microsoft Office, Word, Excel and Outlook and PowerPoint.
• Excellent presentation and communication skills.
• Clinical Background with ability to conduct Patient Assessments.
• Ability to work under stress and to respond quickly in crisis situations.
• Knowledge of healthcare marketing and plan development.
• Ability to work cooperatively as a member of a team.

• Must read, write and speak fluent English.
• Must have good and regular attendance.
