Job Summary
We are seeking a compassionate and skilled Per Diem Home Health OASIS Registered Nurse (R.N.) to join our team of dedicated healthcare professionals.
Responsibilities
- Conduct comprehensive assessments of patients in their homes, identifying their needs and developing individualized care plans to meet those needs
- Provide skilled nursing care to patients, including wound care, medication management, and education
- Collaborate with other healthcare professionals, including therapists, social workers, and home health aides to ensure comprehensive care
- Maintain accurate and detailed documentation of patient care, including OASIS assessments and progress reports
- Communicate regularly with patients, families, and healthcare teams to ensure continuity of care and address any concerns
- Participate in ongoing education and training to stay current with industry developments and best practices
