AccordCare Home Health is seeking skilled Per Diem Field RNs to support Private Duty Home Care patients. Ideal candidates should have current OASIS experience and a passion for providing exceptional care.

What You’ll Do

• Deliver safe, high-quality care based on individualized Plans of Care
• Communicate promptly with the team regarding patient status and changes
• Provide skilled nursing (e.g., wound care, infusion, catheter care, medication administration)
• Partner with Nurse Supervisor/Manager of Clinical Practice to ensure optimal patient outcomes
• Show empathy, professionalism, and excellent follow-through with patients and families

What We’re Looking For

• Current RN license in Florida
• Home Health Experience, or Willingness To Learn
• OASIS experience, or willingness to learn
• Strong clinical skills
• Excellent communication, collaboration, and documentation skills
• Familiarity with WellSky/Kinnser/AlayaCare is a plus
