Peer Support Specialist - Lantern

Responsibilities

• Aid and support consumers during transitions from crisis, inpatient or medical settings to outpatient treatment.
• Collaborate with clinicians, Qualified Personnel (QP), or discharge planners to develop and execute treatment plans.
• Implement intervention strategies aligned with the treatment plan for consumers and their supports.
• Incorporate consumers into activities designed to enhance various skill sets, manage symptoms, educate on wellness, and prepare for work when applicable.
• Maintain ongoing contact with consumers to ensure smooth transitions between services.
• Provide or assist with transportation to initial appointments.
• Ensure accurate and timely submission of required documentation and assistance with service authorizations as necessary.
• Preserve confidentiality and adhere to service guidelines.
• Perform other duties and responsibilities as directed by the immediate supervisor.
