Peer Navigator

About the role

Join us in our mission as a Peer Navigator at Catholic Charities of Care Coordination Services. You will play a pivotal role in individuals' lives, making a significant impact through:

• Make initial contact with eligible participants within 3 hours of receiving referral
• Facilitate initial intake with referrals
• Deliver outreach and engagement services in accordance with the service plan
• Consult with case management staff, Health HUB staff and/or participant’s providers on progress of service/care plan services
• Conduct prevention related harm reduction/risk reduction interventions with participants
• Provide linkage and navigation to needed services for family members and other identified support systems
• Maintain case records and documentation in accordance with agency and OASAS standards

Qualifications

• A High School Diploma or GED
• Experience working with a diverse population
• A valid driver’s license, proof of current car insurance and registration, and access to a reliable, working vehicle

Pay

Peer Navigator Minimum Salary: $ 20.51 Peer Navigator Maximum Salary: $ 20.51

Peer Navigator Schedule: Mon - Fri (37.5 hours/week)
