About the role
The LVN provides patient-centered nursing care by collecting data, assisting in developing individualized care plans, and evaluating patient responses.
Responsibilities
- Assessment: Assists in patient history, documents signs and symptoms, safety, and teaching needs.
- Care Planning: Documents problems, goals, and communicates plans with the healthcare team and family.
- Implementation: Executes delegated care, initiates referrals, explains procedures, and documents interventions.
- Evaluation: Assesses patient outcomes, collaborates on goal attainment, and revises care plans accordingly.
- Professional Growth: Accepts new responsibilities, maintains standards, manages workload, and supports team development.
- Guest Relations & Interdepartmental Collaboration: Participates in communication, conflict resolution, and maintains a therapeutic environment.
- Performance Improvement: Engages in quality initiatives, suggests efficiencies, and participates in corrective actions.
- Knowledge & Treatment Range: Understands growth/development, manages various patient conditions, and applies unit-specific competencies.
Qualifications
- Minimum one year skilled nursing experience
- Current LVN license
- BLS and PALS certifications required
- Preferred experience in acute care
