Pay Range: $30.70 – $46.05 per hour
Responsibilities
- Interview patient and patient’s social support system to complete initial psychosocial evaluation.
- Provide counseling, support, and education to patient and patient’s social support system.
- Assist patients and their social support system in coping with stress related to hospitalization, disability, chronic or terminal illness.
- Collaborate with patients, their social support system, and the multidisciplinary team to establish and implement a plan of care addressing identified psychosocial needs.
- Serve as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient’s illness and its effects on their support system.
- Facilitate the multidisciplinary team’s understanding and integration of psychosocial aspects into the plan of care.
- Intervene in crisis situations, applying crisis intervention theory and skills effectively.
- Complete psychosocial assessments for post-hospital care for designated patients and their support systems.
- Work with the Clinical Care Management team to assess and evaluate the appropriate level of care or environment setting to meet care needs across the continuum.
- Participate in multidisciplinary care planning meetings.
- Maintain current knowledge of and research availability of community agencies and resources for social, emotional, or financial assistance.
- Provide information and education to patients and their social support on community resources and post-hospital care options, in collaboration with the Clinical Care Management team.
- Make referrals to community agencies as needed.
- Report suspected cases of child and adult abuse, neglect, or exploitation.
- Serve as liaison between the hospital, patients, their social support system, and the Department of Social Services during evaluation or investigation.
- Identify and record clinical, psychosocial, and financial barriers to a smooth transition across the healthcare continuum; assist in identifying and facilitating system improvements.
- Work with Clinical Care Management leadership to identify problems, recommend solutions, and work toward resolution.
- Develop and maintain accurate case records for each referred patient; document in the medical record according to departmental standards.
- Provide education and support services to patients, families, and staff.
- Provide psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis to patients and families.
- Collaborate with the Clinical Care Management team to effect positive patient and family outcomes and ensure efficient utilization of resources.
Requirements
- Master’s degree in Social Work from a program accredited by the Council on Social Work Education required.
- LCSW preferred.
- If working in South Carolina, must meet and maintain state licensure requirements.
- At least 1 year of professional experience in a hospital or health-related setting preferred.
- Expertise with data management tools.
- Adherence to the National Association of Social Workers Code of Ethics.
Physical Requirements
- Works in an office setting with extensive walking throughout the facility.
- Prolonged periods of sitting while reviewing medical records and documenting.
- Repetitive wrist motion and occasional lifting of 10–20 pounds.
- Requires frequent verbal and written communication in English.
- Intact sight and hearing, with or without assistive devices.
- Must be able to handle a fast-paced environment and move independently from one location to another.
