Responsibilities
- Actively monitors and amends patient visit schedule to maximize productivity and patient need.
- Acts to account for new patients and changes in existing patient schedule, including but not limited to, calling patients to reschedule visits, reshuffling patient load to meet new demands, and communicating recommended changes to supervisor.
- Performs a comprehensive PT assessment of assigned patients, including psychosocial, educational, safety, and risk factor parameters, as evidenced by clinical documentation.
- Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
- Plans for and provides skilled physical therapy treatment, including therapeutic techniques, patient/caregiver education, etc., assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
- Coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, caregivers, other rehab services, and students, ensuring optimal patient care and communication as noted in the clinical record.
- Fills out and completes insurance paperwork in a timely manner.
- Serves as a Home Care PT as required.
Qualifications
- Bachelor's degree in Physical Therapy from an accredited university or college required.
- Master's degree preferred.
- Vermont State Physical Therapist license required.
- Valid State Driver's license required.
- Ability to successfully pass pre-employment driver record, criminal, adult, and child abuse background check/review required.
