Responsibilities

• Provide a full range of physical therapy services to patients in their homes, including ongoing assessments of current status; implementation of treatment plans, including specific therapeutic exercises/activities and instruction in using adaptive techniques and equipment to regain maximum independence.
• Teach patients and caregivers about their conditions and PT treatment plans, self‑treatment and care.
• Confer regularly with supervisor, other clinical team members, departmental office staff, and other UVMHN Home Health & Hospice personnel to plan and coordinate activities, consult on cases, exchange information, investigate and resolve problems.
• Perform other duties as assigned.
