About the Role

The Nurse Coordinator is a Registered Nurse who reports to Nursing leadership within the Pediatric Specialty Centers and works under the direction and supervision of the Attending Physician for the specific specialty or program. This role ensures seamless coordination of patient care across the continuum, including clinical practice, care coordination, education, and quality improvement, with significant latitude for exercising professional judgment. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally appropriate care to patients and families within the specialty service.

Responsibilities

  • Professional Practice: Lead member of the healthcare team coordinating all aspects of patient care for the specific specialty.
  • Education: Provide patient and family education on the medical condition, including signs, symptoms, treatment regimens, side effects, expected outcomes, potential adverse reactions, and condition management to promote optimal health.
  • Multi-Disciplinary Collaboration: Function as a key member of the multidisciplinary team, coordinating all aspects of patient care across the continuum.
  • Quality Improvement: Participate in data collection related to clinical and operational efficiencies.

Requirements

  • Registered Nurse with a Connecticut RN license.
  • Bachelor’s Degree in Nursing preferred.
  • At least 2-3 years of RN experience in an inpatient or outpatient setting required.
  • Relevant experience in the specialty area, pediatrics, or in the Nurse Coordinator role preferred.

Skills

  • Excellent interpersonal and organizational skills.
  • Ability to assert oneself, advocate for patients, and work independently with minimal supervision.
  • Ability to manage multiple priorities.
  • Computer experience, including word processing and data entry systems.
  • Strong communication skills with all members of the healthcare team, patients, and families.
  • Commitment to a "patient-centered model of care."

Physical Demands

Must be flexible to meet the demands of the specialty, outpatient clinic sessions, and patient/family needs.
