General Purpose: Provide each assigned resident with routine daily care and services in accordance with the resident’s assessment and care plan, as directed by supervisors.
Responsibilities
- Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
- Report all changes in residents’ condition to the Health and Wellness Director as soon as practical.
- Record all entries on notes, SPAs, and incident reports in an informative, descriptive manner.
- Report all accidents and incidents observed on the shift to the Executive Director and Health and Wellness Director.
- Agree not to disclose residents’ protected health information and promptly report suspected or known violations.
- Report any known or suspected unauthorized attempt to access the community’s information system.
- Perform only those care tasks for which you have been trained.
- Ensure residents’ rooms are ready (e.g., bed made) and greet residents upon arrival.
- Introduce residents to roommates and other personnel as appropriate.
- Inventory and mark residents’ personal possessions as instructed; assist with packing/unpacking during transfers or discharges.
- Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
- Maintain an atmosphere of warmth, personal interest, and positive emphasis; ensure a calm environment.
- Meet with nursing personnel regularly to identify and correct problem areas or improve services.
- Report all resident complaints and grievances.
- Participate in and receive nursing reports as instructed.
- Follow established policies concerning exposure to blood/body fluids.
- Perform daily care tasks, including:
- Making beds and ensuring extra covers are available as requested.
- Checking on residents unable to call for help frequently.
- Answering resident calls promptly and ensuring personal care needs are met.
- Assisting with meal arrangements, including informing residents with sight problems about food placement and temperature.
- Keeping water pitchers clean, filled, and within reach.
- Providing after-meal care (e.g., removing trays, cleaning residents).
- Assisting with dental/mouth care, bathing, dressing/undressing, and grooming (e.g., combing hair, shaving, nail care).
- Changing wet or soiled gowns, clothing, and linens.
- Assisting with bowel and bladder functions (e.g., bathroom trips, bedpans, urinals, portable commodes).
- Maintaining intake and output records as instructed.
- Observing and reporting bowel movements and stool characteristics.
- Performing restorative and rehabilitative procedures as instructed.
- Using protective and adaptive equipment as instructed.
- Providing eye and ear care (e.g., cleaning eyeglasses, installing hearing aids).
- Assisting with physical examinations and weighing/measuring residents.
- Measuring and recording temperatures, pulse, and respirations (TPRs) as instructed.
- Assisting with end-of-life and post-mortem care as instructed.
- Lift, turn, move, position, and transport residents using proper techniques and equipment (e.g., beds, chairs, bathtubs, wheelchairs, lifts).
- Report injuries of unknown source, including skin tears, and observe disoriented/comatose residents.
- Perform special treatments as instructed and report defective equipment to the Nurse Supervisor/Charge Nurse.
- Immediately notify the Nurse of any resident leaving or missing from the facility.
- Maintain confidentiality of all resident care information, including protected health information.
- Report allegations of resident abuse and/or misappropriation of resident property.
- Honor residents’ refusal of treatment and report such requests to supervisors.
Requirements
- High school education or equivalent (minimum).
- Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to questions from managers and employees.
- Proficiency in applying mathematical concepts (e.g., fractions, percentages, ratios) to practical situations.
- Ability to solve practical problems with limited standardization and interpret instructions in written, oral, diagram, or schedule form.
- Proficiency in Microsoft Suite products.
Physical Demands
- Occasionally lift and/or move up to 25 pounds.
- Prolonged use of desktop or laptop computers.
- Regularly required to sit, stand, walk, talk, read, or hear.
- Frequent use of office-related equipment (e.g., copier, scanner, fax, telephone, calculator).
Work Environment
- Low to moderate noise levels.
Pay
Starting at $19/hour, depending on experience.
