Patient Transport Driver - Gretna Pace/Full Time Days

About the role

This position provides transportation services for Centra PACE/Ground Transport under the direction of the Transport Supervisor(s). The Driver will transport PACE participants and patients to and from Centra facilities, ensuring a safe and enriching experience from door to door.

Responsibilities

• Provide exceptional door-to-door care to all PACE participants and Centra patients.
• Communicate all participant/patient changes and needs to the direct supervisor.
• Perform pre- and post-vehicle safety inspections, ensure the vehicle is in proper operational condition, and report any maintenance issues to the immediate supervisor.
• Maintain cleanliness of the interior and exterior of Centra vehicles.
• Follow scheduled transports for PACE participants or patients and other potential trips communicated by the direct supervisor.
• Maintain all necessary records and documentation regarding participants, patients, and vehicles.
• Enter safely into participants’ homes on a routine basis.
• Participate as an extension of the Transport Supervisor as a member of the interdisciplinary team and in participants’ care plans.
• Deliver participant medications, medical supplies, and other medical necessities.
• Perform other duties as assigned.

Requirements

• Valid driver’s license in the United States.
• Must meet a standardized set of competencies for the specific position established by Centra PACE and regulatory agencies.

Pay

$15.90 - $22.26 per hour.
