Patient Service Coordinator - Non-Clinical

Responsibilities

• Schedules and performs patient assessment visits including all Oasis visits timely.
• Schedules and assigns clinicians in the most efficient geography, maximizing clinician efficiency, utilizing the lowest possible discipline, matching skill with required care, and optimizing clinician capacity.
• Collaborates with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
• Manages calls from patients and field staff related to scheduling issues.
• Refers clinical and performance related issues to the clinical manager.
• Maintains communication with hospitalized patients, ensuring the team follows up as necessary.
• Supports the clinical manager and collaborates to ensure timely communication with patients, clinicians, referral sources, business development team, and other office personnel.
• Runs applicable reports and takes appropriate actions based on work flow.
• Affords assistance in transferring patients between care centers and/or hospice services.
• Maintains supply closet with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serves as liaison with the field for patient supply needs.
• Performs other duties as assigned.

Qualifications

• A High School diploma or GED equivalent.
• Prior scheduling experience in a healthcare environment.
• Prior administrative experience in a healthcare environment.

Pay

$21.00 to $25.00 per hour, depending on home health experience.
