Responsibilities

• Guide patients through the health care system by assisting with access to care issues.
• Develop relationships with service providers.
• Track interventions, outcomes, and follow-up.
• Educate patients and families on diagnosis and recommended treatment options throughout the continuum of care, including surgical interventions, procedures, ancillary tests, and post-discharge follow-up.

Qualifications

• Three years of nursing experience in an acute care setting (required).
• Bachelor’s degree (required).
• Registered Nurse (required).
• Oncology Certified Nurse (desired).
