Major Duties And Responsibilities
- Communicate clearly, respectfully, and compassionately with patients and caregivers
- Accurately document medical data and patient communications
- Communicate clearly with all members of the care team within the organization and to external partners in order to convey pertinent patient issues, appointment information, provider schedules, and other information
- Assist patients with insurance enrollment using the Maryland Healthy Babies program
- Identify and track pregnant patients and newborns at Chase Brexton Health Care Glen Burnie and Columbia Centers and follow up on missed appointments
- Assess patients for barriers to care and coordinate linkage to internal and external resources
- Retrieve clinical documentation from multiple sources to ensure continuity of care for patients
- Provide education, support, and outreach to retain pregnant patients in prenatal care throughout pregnancy and postpartum; and engage newborns in pediatric care
- Engage with pregnant patients and newborns utilizing multiple modes of outreach including attendance at community events (possibly on weekends) with the Program Manager
Skills And Abilities
- Excellent customer service skills
- Excellent interpersonal skills
- Demonstrated strong verbal communication skills
- Knowledge of HIPAA and OSHA regulations
- Familiarity with medical terminology
- Proficient in Microsoft Office Outlook, Word, and Excel
- Ability to work with multiple healthcare computer applications
- Ability to learn quickly
- Ability to adapt to change
- Bilingual in English and Spanish (optional)
Education And Experience
- Required: Some college or equivalent work experience
- PREFERRED: Degree in health sciences or related field
- Experience Required: One year experience working in the medical field
- Experience working with a culturally and economically diverse population
